Care Coordinator - LVN/LPN

The Care Coordinator works from the MHT's collaborating physician office with the Provider and together form the Care Team. The Care Coordinator is responsible for coordinating the following; establishing and promoting MHT with the physician, staff and referring community; responsible for the overall productivity and profitability of the MHT practice; insurance verification and entry of and obtaining demographics, incoming and outgoing activities for the Care Team; preparing daily schedules, coordinating visits with caregivers and/or family members; handling orders, managing documents, care plan oversight (CPO), and Care Coordination Management, billing, making follow-up phone calls for patient monitoring, ensuring patient's receive information timely; and being the liaison with all referring and providers agencies; Transitional team and at times inclusive with Chronic Care Team obtain information from hospital or skilled nursing facility, scan report into the EHR, verify insurance, enter demographics, follow-up phone calls within 48-72 hours of transition patient for compliance with medication and status of condition. This an hourly position with hours 8:
00 AM to 5:
00 PM or whatever is needed to complete responsibilities.
POSITION QUALIFICATIONS
A graduate from an accredited LVN/LPN nursing program with current licensure.
Must have a minimum of two years experience in a health care setting utilizing clinical knowledge combined with strong administrative skills.
Must have knowledge of Medical Terminology.
Ability to perform the functions of medication refills, labs, diagnostics, Document Management, triage calls, and ordering of equipment and supplies, etc.
For the Transitional Care Coordinator, the LVN/LPN will work and assist patients with the transition from healthcare agency to home. Assist with financial needs and finding supportive services.
Ability to articulate clearly and possess excellent command of oral and written skills.
Must possess excellent listening skills and the ability to ask probing questions, understand concerns and overcome objections.
Ability to be adaptable to a fast moving environment and use good judgment in all situations.
Ability to conduct problem analysis and problem solving.
Ability to work as a team member and foster a team spirit.
Knowledge of Microsoft Office with good word processing/typing speed, spelling skills and knowledge of patient records (EHR) a must.
Ability to keyboard at least 45-50 words per minute and to develop and enter information into an Excel spreadsheet.
Ability to speak clearly and comfortably and use tact, discretion and sensitivity in dealing with patients, and to deal courteously and efficiently with physicians, Providers, office staff and others.
Bilingual in Spanish, preferred
Reliable transportation to get to and from work with required automobile insurance.
Ability to work in a smoke-free environment.
